Root Canal vs. Extraction: Which Is Better?

When you're dealing with severe tooth pain or infection, choosing between a root canal and a tooth extraction can feel overwhelming. While both options aim to relieve discomfort and restore oral health, a root canal is typically the preferred choice because it preserves the natural tooth and can prevent long-term complications. At Smith & Smith Endodontics, we guide patients through this important decision, helping them understand the benefits of saving a tooth whenever possible. If you're unsure which treatment is right for you, our Florence, AL team is here to provide the clarity and care you need.

What is a root canal procedure?

A root canal involves removing the infected or inflamed pulp from inside the tooth. Once cleaned and disinfected, the space is filled and sealed, and the tooth is later restored with a crown or filling. The goal of a root canal is to get rid of infection while preserving the natural tooth, which helps maintain the structure of your smile and bite.

What is a tooth extraction?

A tooth extraction removes the tooth from the socket. This may be necessary when the tooth is too decayed or damaged to save. While extraction quickly removes the source of pain and infection, it often leads to the need for additional dental work — such as a bridge, denture, or implant — to restore function and prevent surrounding teeth from shifting.

Is it better to get a root canal or an extraction?

Choosing between a root canal and a tooth extraction depends on more than just the condition of the tooth — it also involves considering your long-term oral health, treatment goals, and lifestyle. A root canal is often the better option because it allows you to keep your natural tooth, maintain proper bite alignment, and avoid the added time and expense of tooth replacement options.

While to some an extraction may seem like a quicker fix, it can lead to additional procedures like dental implants or bridges to restore function. Preserving your natural tooth through root canal treatment typically results in a more stable and cost-effective solution over time, making it the first choice for most patients when the tooth can be saved. Our board-certified endodontists will work with you to ensure you understand the full impact either option will have on your oral health.

What are the benefits of a root canal?

Choosing a root canal over extraction offers several long-term advantages for oral health and overall comfort. Some benefits of root canal therapy include:

  • Saves natural tooth
  • Maintains alignment and prevents jawbone loss
  • Offers lasting tooth pain relief
  • Avoids the need for prosthetic replacements
  • Quick recovery and long-term function with proper restoration

When is a tooth extraction necessary?

While saving the natural tooth is usually the preferred option, there are times when a tooth extraction may be the better or only solution. If the tooth is severely decayed, fractured beyond repair, or has extensive bone loss due to infection, it may not be viable for a root canal. In some cases, a crack that extends below the gumline compromises the structural integrity of the tooth, making restoration impossible.

Other factors — such as medical conditions or repeated root canal treatments that have failed — may also make tooth extraction the more practical choice. Tooth extraction is not taken lightly; if it's decided as the more viable treatment, planning for a replacement option like a dental implant or bridge is key to maintaining oral health and function after extraction.

I had been in pain for 3 weeks with some new crowns. Turns out I needed a root canal. Dr. Wilder Smith was outstanding! I came home and didn’t even need anything pain relief wise, which was huge for me! He’s humorous - while working on you and very gentle. All ladies in office were wonderful as well. A+ Dr. Smith! Highly recommend!

Went in to have two root canals done and they were all very nice and even worked through their lunch to fit me in.

I came to see Dr. Wilder Smith after a negative experience with another dentist. I could not be more pleased!!! I was very nervous to have a root canal, since I've never had one before and had just come from a negative experience getting a crown with another dentist. Dr. Smith acknowledged my anxiety and was so kind and funny, cracking jokes throughout the whole procedure to keep me at ease. All of the other staff members were super calming and pleasant as well. I came in with pain and extreme sensitivity to even room temperature water. I left feeling amazing. They advised me to take an ibuprofen before the numbing wore off, but to tell the truth, I did not take an ibuprofen, and have not taken one since having the procedure. I left with no pain or sensitivity at all, just couldn't chew for a couple days but that's to be expected! I seriously cannot thank them enough. I was miserable for a month before seeing them and I finally feel like I'm back to my normal life.
